29 CFR 1926.304(d): All portable, power-driven circular saws were not equipped with a guard that; when the tool was withdrawn from the work the lower guard did automatically and instantly return to the covering position:  (a) 2 Wisteria Ln East Longmeadow, MA: On or about March 11, 2014, the lower blade guard on the Makita brand circular saw was wired fixed in the open position so as to render the blade unguarded, which is not in accordance with this subpart:  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level:    2 Wisteria Ln East Longmeadow, MA: On or about March 11, 2013, an employee was installing siding from the scaffold at this address more than 19 FT above the ground below without any fall protection, which is not in accordance with this subpart:    Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.452(j)(2): Pump jack scaffold poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and/or other points as necessary:    (a) 2 Wisteria Ln East Longmeadow, MA: On or about March 11, 2014, an employee was working from a pump jack scaffold system that was only attached to the structure at the top of the poles, which is not in accordance with this subpart.
