29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: (Construction Reference 1926.103) (a) The employer did not implement a written respiratory protections program with procedures for selecting respirators, medical evaluations for users, fit test-testing, training on respiratory hazards such as diisocyante, and procedures for regularly evaluating the effectiveness of the program for employees required to wear 3M 8511 N-95 cloth filtering facepieces while applying Handi-Foam two part spray insulation containing 30 to 60 percent diisocyanate.
