29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift.   On or about March 10, 2014, on the west side of the construction site, an employee installing TYVEK material from a S-45 Genie man-lift was exposed to falling more than30 feet to the lower level.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.    On or about March 10, 2014, on the west side of the commercial construction site, employees using an extension ladder to access the roof without the side rails extending 3 feet above the landing were exposed to a fall hazard of approximately 30 feet to the ground below.
