29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. On or about March 11, 2014, on the west side of the building, employees were not wearing hard hats while working below other employees that were engaged in decking activities. The employees were exposed to struck-by hazards from falling objects.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about March 11, 2014, on the west side of the building, employees engaged in decking activities without fall protection were exposed to a fall hazard of approximately 10 feet to the lower level.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about March 11, 2014, on the west side of the construction site, employees used an extension ladder that did not extend 3 feet above the landing were exposed to a fall hazard of approximately 10 feet to the ground below.
