29 CFR 1926.1053(b)(1):   Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet  above the upper landing surface to which the ladder was used to gain access:   A) Front of house: Employees were exposed to fall hazards of approximately 22 feet while using an extension ladder to access the roof of a house. The extension ladder that was used did not extend at least 3 feet above the landing, on or about March 11th, 2014.

29 CFR 1926.1053(b)(16):   Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, shall either be immediately marked in a manner that readily identifies them as defective, or be tagged with "Do Not Use" or similar language, and shall be withdrawn from service until repaired.   A) 241 Ridge Road, Rutherford- NJ: Employees were exposed to fall/ladder collapse hazards of approximately 22 feet while using an extension ladder with 2 broken feet, on or about March 11th, 2014.
