29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:   a.  During maintenance work on the CDI poly lamination machine, disconnects were not turned off and the machine was not locked out.   Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).

29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items of this section:  a.  Where lockout work was performed, such as maintenance of the corrugated poly lamination machines, including clearing jams, lockout procedures had not been developed and written.  (Note: alternative measures which provide effective protection during minor servicing activities and adjustments could be included in the written procedures.)  Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:  a.  Where lockout work was performed, such as maintenance of the corrugated poly lamination machines, including clearing jams and performing minor servicing, lockout training had not been provided according to the requirements of the standard, including informing employees whether they were authorized to lockout or not, reviewing written lockout procedures, ensuring how and where to lockout, such as the specific energy isolating device(s), and how to test a machine after applying the lockout device.  (Note: all the elements of training must be included in training (1910.147(c)(4)(ii)(A) through (D).)  Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).
