29 CFR 1910.215(a)(4): Work rests were not kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage: In the maintenance facility shop on or about March 13, 2014, and at times prior thereto, the work rest was not adjusted to within one-eight inch of the abrasive wheel as it was three-eights in from the wheel on a Baldor Grinder Cat. # 762, serial number W0705091650. The grinder was used to sharpen knives to cut boxes.
