29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated. (a) While working inside the basket of an aerial lift at the Family Dollar in Savannah, GA, on or about March 13, 2014, no fall arrest system or other forms of fall protection was in use by two employees who were working near the roof line of the structure, resulting in an approximately 15-foot fall hazard.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. (a) While installing roof decking and the associated components at the Family Dollar in Savannah, GA, on or about March 13, 2014, no fall arrest system or other forms of fall protection was in use by two employees who were working from structural roof beams of the structure, resulting in an approximately 20-foot fall hazard.
