29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:    JOB SITE: 336 W. Washington Avenue, Kirkwood, MO 63122, on or about 3/11/14, at this work site, the employer failed to conduct frequent and regular inspections of the jobsite by a competent person.  The foreman failed to recognize fall protection requirements on a 6:12 pitch roof and provide eye or face protection while operating power tools.  The workers were exposed to an eye, face and fall hazard.

The worker was exposed to a struck-by hazard.
