OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees  (a)  On or about March 11, 2014, the employer did not have the automotive lifts periodically inspected by a qualified inspectors at least annually to ensure that they were properly maintained and functioning.  Employees were potentially exposed to crushing and struck-by hazards while operating automotive lifts. 29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  (a) On or about March 11, 2014, the employer exposed employees to stuck-by and caught-in hazards due to the three bench grinders work rests not being adjusted to one eight inch;  (1) A Blue Point bench grinder, located in the Rebuild Room, had a work rest which was in excess of one eight inch.  (2) A Dayton bench grinder, located in the Rebuild Room, had a work rest which was in excess of one eight inch.  (3) A Blue Point bench grinder, located in the Main Removal and Replace Room, had a work rest which was in excess of one eight inch.
