ABATEMENT CERTIFICATION AND DOCUMENTATION ARE NOT REQUIRED

a) At the job site - on or about March 13, 2014, two employees were working from a tubular welded frame scaffold without guardrails and no other means of fall protection, exposing employees up to a 21-foot fall hazard. LB Masonry was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1), which was contained in OSHA inspection number 314090309, citation number 2, item 001b and was affirmed as final order on January 5, 2011, with respect to a workplace located at 6295 US Hwy 319, Thomasville, Georgia. ABATEMENT CERTIFICATION AND DOCUMENTATION ARE NOT REQUIRED
