29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide. (a) Building 18, Southeast Side - On or about March 11, 2014 one employee was exposed to fall hazard when installing siding from a pump jack scaffold. The working platform was 14 inches wide.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. (a) Building 18, Southeast Side - On or about March 11, 2014 one employee was exposed to fall hazard when installing siding from a pump jack scaffold. Employees were working 28 feet above a lower level. Employee was not protected from falling to a lower level by a guardrail system, personal fall arrest system or safety net.
