29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   a.  On or about March 12, 2014, an employee working on the ground handing up material and equipment to employees on mobile scaffoldings was not wearing a hard hat, exposing him to overhead hazards.

29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary:    a.  On or about March 12, 2014, underneath the covered area, the mobile scaffold system was missing planks on the second level where the employee were installing screws into the drywall in the ceiling, exposing him to a 10 feet 8-inch fall hazard.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcture, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access:    a.  On or about March 12, 2014, the mobile scaffold system did not have a ladder or other safe means of access, exposing employees to a fall hazard.

On or about March 12, 2014, the blue mobile scaffold did not have guardrails, exposing employees to a 10 feet 8-inch fall hazard.  b. On or about March 12, 2014, the yellow mobile scaffold did not have guardrails, exposing employees to an 11 feet 2-inch fall hazard.
