29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.   The employer does that each operator is trained formal instruction and practical training. This violation occurred on or about March 6, 2014, in the Unloading Area where employees were exposed to a struck-by hazard when an employee was allowed to operate a forklift without formal instruction and practical training.   Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that before employees operate a power industrial truck they receive formal instruction and practical training.
