29 CFR 1910.95(g)(1): The employer did not as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:  On or about January 15, 2016, in the production area, where employees were exposed to hazardous noise levels while operating machinery without an established and maintained audiometric testing program.

29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.  On or about December 3, 2015, in the production area, where employees were exposed to caught in and struck by and electrocution hazards when performing service and maintenance on machinery without specific steps to control hazardous energy.
