29 CFR 1926.102(a)(1):   Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents.  On 3/10/2014, workers from Juan Garcia Roofing were repairing a roof of a residential building located at 6700 Armstead Court, Barnhart MO.  Three employees were not wearing eye protection while using pneumatic nailers.

29 CFR 1926.501(b)(13):   "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system.  On 3/10/2014, workers from Juan Garcia Roofing were repairing a roof of a residential building located at 6700 Armstead Court, Barnhart MO.  They were not utilizing guardrail systems, a safety net system, or personal fall arrest systems.  The foreman, Mr. Santies, and seven employees were observed on the roof of the building without fall protection, exposing him to fall to the ground below.

29 CFR 1926.1053(b)(1):   When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access.  On 3/10/2014, workers from Juan Garcia Roofing were repairing a roof of a residential building located at 6700 Armstead Court, Barnhart MO.  They were using a ladder improperly to access the roof.  The ladder was not extended to three feet beyond the landing or edge of the roof.

29 CFR 1926.1053(b)(16):       Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.  On 3/10/2014, workers from Juan Garcia Roofing were repairing a roof of a residential building located at 6700 Armstead Court, Barnhart MO.  They were using a defective portable extension ladder, no serial number, to access a residential building to repair the roof.  The ladder had a bent railing, several damaged rungs, and a damaged or bent foot.
