29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a.   On or about March 13, 2014, on the northeast side and middle of the structure, employees installing metal roofing materials to a 5/12 pitch roof were not protected from a 10 feet 4 inch fall hazard by the use of a personal fall arrest system.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a.   On or about March 13, 2014, on northeast side of the house, the ladder used to access the roof did not extend 3 feet above the upper landing surface, exposing employees to a 11 feet 4 inch fall hazard.
