29 CFR 1926.600(a)(6)(i): 29 CFR 1926.600(a)(6)(i): For lines rated 50kV or below, a minimum of 10 feet was not maintained between the energized line and the equipment: a. On or about March 6, 2014, at the above addressed jobsite, employees were exposed to an electric shock while operating a Caterpillar backhoe loader and installing metal fence poles along a golf course perimeter in close proximity to overhead high voltage electrical power lines that were not de-energized, grounded or guarded.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a. On or about March 6, 2014, at the above addressed jobsite, employees who were not instructed in the recognition of and avoidance of electric shock were using a Caterpillar backhoe loader to install metal fence poles along a golf course perimeter in close proximity to overhead electrical power lines.
