The Electric Materials Company, 50 South Washington Street, North East, PA-North Hold Area of the Mill, on or about March 12th, 2013, and times prior thereto: Company employees operated a Komatsu #2395 (Serial # 373342A) industrial truck daily and/or over multiple work shifts. The Komatsu #2395 industrial truck was not being examined after each work shift.

The compressed air at the North Hold Area of the Mill was set at greater than 30 p.s.i.
