The workers were exposed to a struck-by hazard.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection: JOB SITE: 855 Newport Avenue, Webster Groves, MO 63119 on or about 3/13/2014, at this work site, workers were observed using a 120v, single phase receptacle to power their radio, which was not part of the permanent wiring of the building and did not have an approved ground fault circuit interrupters (GFCI). The workers were exposed to an electrical shock, burn hazard.

29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage: JOB SITE: 855 Newport Avenue, Webster Groves, MO 63119 on or about 3/13/2014 at this work site, observed a 120v single phase receptacle and cord set which was used to power their portable hand tools and radio, was missing a ground pin and the insulation was pulled away from the receptacle. The workers were exposed to an electrical shock or burn hazard.
