One platform over the clear well area did not have guardrails on its end. This exposed the employees to the hazard of falling over 4 feet.

29 CFR 1910.106(e)(2)(ii)(b)(2): The quantity of Category 2, 3 or 4 flammable liquid located outside of an inside storage room or storage cabinet in a building or in any one fire area of a building exceeded 120 gallons: Cooler interior storage room: On or about March 12, 2014, employees engaged in mixing soft drink ingredients accessed the cooler interior storage room to obtain flammable liquid ingredients. The flammable liquid quantities exceeded 120 gallons and the room did not meet the requirements for a flammable storage room. This exposed the employees to the hazards of fire.

The machines had inadequate guarding in the following instances exposing the employees to the hazards of catch and pinch points. a) The rotating filling carousel with descending fillers fixtures. b) The rotating enclosure carousel with descending enclosure fixtures. c) The rotating discharge carousel.
