29 CFR 1926.350(a)(7): A suitable cylinder truck, chain, or other steadying device was not used to keep compressed gas cylinder(s) from being knocked over while in use: a) Facility, Office-side: On or about March 11, 2014, an employee was conducting mechanical installation work while using an unsecured acetylene cylinder set on a rolling cart that was only partially restrained by the fuel gas hose, creating a tip over condition and loss of the cylinder's gas and liquid contents.
