29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    Location:  744 South 3rd Street, St. Louis, MO  63102    At the job site, a worker was not wearing a protective helmet while exposed to struck-by hazards.  The worker was performing masonry activities on the ground directly below the tuckpointer working on the ladder jack scaffold.

29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage:     Location:  744 South 3rd Street, St. Louis, MO  63102    At the job site, the employer failed to ensure the all electrical power equipment (power tools & flexible cords sets) were visually inspected for external defects prior to use.  Visual inspection of the flexible cords revealed insulation damage and a missing ground pin.  The workers were exposed to an electrical hazard.

29CFR1926.404(f)(6): The path to ground from circuits, equipment and enclosures were not permanent and continuous:  Location:  744 South 3rd Street, St. Louis, MO  63102  At the job site, the path to ground from circuits was not permanent and continuous when workers used an electrical flexible cord to power portable hand grinders to remove brick mortal with a missing ground pin.  The workers were exposed to an electrical hazard.

29 CFR 1926.451(e)(1): An access ladder or equivalent safe access to scaffold was not provided:    Location:  744 South 3rd Street, St. Louis, MO  63102    At the job site, the ladder jack scaffold on the west facing side of the building protruded out from the structure was not provided with safe access.  The employee was exposed to a fall hazard as he climbed over the outward facing ladder jack bracket to access the scaffold plank.  The distance between the scaffold plank and the ground measured approximately 30-feet, 7-inches at the north side of the plank and 31-feet, 3-inches at the south side of the plank.
