29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, or uprights did not bear on base plates, mud sills or other adequate firm foundation. The employer does not ensure that scaffold poles, legs, posts, frames, or uprights bear on base plates, mud sills or other adequate firm foundation. This violation occurred on or about March 13, 2014 when an employee was exposed to a fall hazard while doing masonry work from the second level of a scaffold that did not bear on base plates, mud sills or other adequate firm foundation. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all employees working from a scaffold in which the poles, legs, or frames bear on base plates, mud sills or other adequate firm foundation.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. The employer does not ensure that employees are protected from falling when accessing a scaffold platform. This violation occurred on or about March 13, 2014 where an employee was exposed to a fall hazard while access a scaffold platform that was more than 2 feet above a point of access by using the crossbraces as a means of access. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees have proper access to the scaffold platform.
