29 CFR 1910.212(a)(3)(ii):  Points of operation of machinery were not guarded to prevent employees from having any part of their body in the danger zones during operating cycles:  Employees working to form parts on the tandem Cincinnati Hydraulic Presses were exposed to amputation hazards and employees were not protected from contact with the die and the point of operation was not guarded to prevent employees from reaching into the die during an operating cycle. Parts were being formed by two employees and each employee was not provided two hand controls or another form of guarding that would prevent the employee from having any part of their body in the danger zone, and the area behind the tandem Cincinnati Hydraulic Presses are not guarded to prevent employee access.
