29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. (a) Roof of Housing, Lot No. 2 - On or about March 12, 2014 three employees were using pneumatic air nail gun to install roof sheathing. Employees were not wearing safety glasses. Employees were exposed to being struck by flying debris or nails.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) Roof of Housing, Lot No. 2 - On or about March 12, 2014 four employees were exposed to a fall hazard when working on a roof greater than 6:12 pitch roof with unprotected sides. Employees were exposed to falling 12 feet to a lower level. Employees were not protected from falling to a lower level by the use of personal fall arrest system, safety net system or guardrail system.

29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support that do not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder. (a) Roof of Housing, Lot No. 2 - On or about March 12, 2014 four employees were exposed to a fall hazard when ascending and descending from a roof on ladders which were not extended at least 3 feet above the upper landing surface. The ladders were approximately 11 feet long. Ladders were not secured and a grasping device was not provided.
