29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift. On March 13, 2014, two employees were working on a man-lift, one in the basket without using a body belt or a harness and the other was working below. This condition exposed one employee to falls greater than 4 feet to the ground below and other to struck-by hazards.
