29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables shall be protected from damage. Sharp corners and projections shall be avoided. Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage.  a. In apartment 420 where employees ran an extension cord through a window to the roof, the extension cord was not protected from damage, thereby exposing employees to an electrical contact hazard.

On the roof of the apartment building where employees were sealing roof seams, warning lines were not erected around all areas where employees were working, thereby exposing employees to a 40 foot fall hazard.

29 CFR 1926.502(f)(1)(iii): Points of access, materials handling areas, storage areas, and hoisting areas shall be connected to the work area by an access path formed by two warning lines.  a. On the roof of the jobsite where employees were sealing roof seams, the warning line that was set up around the access hatch directed employees off edge of the roof, thereby exposing the employees to a 40 foot fall hazard.

29 CFR 1910.1200(h)(3)(iv): The details of the hazard communication program developed by the employer, including an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employees can obtain and use the appropriate hazard information.  a. On the roof of the jobsite where employees used chemicals to secure and seal the roof, such as but not limited to weathered membrane cleaner, employees were not trained on the HAXCOM 2012 requirements referencing safety data sheets, new label elements, pictograms, and signal words, exposing employees to contact hazards.
