Location: Manufacturing floor a) Employees troubleshooting a web-break on Coater #64 were exposed to amputation hazards while operating the Coater without using lock out procedures; on or about 02/11/14. b) Employee performing routine maintenance on Coater #64 was exposed to an amputation hazard while not using lock out procedures; on or about 02/21/14.
