Jupiter Rd., Garland, TX; where the flexible cord used to power Makita saw did not have strain relief device causing a transmission of tension to joints and or terminals.

Jupiter Rd., Garland, TX; where at least least five (5) employees, who were working on the roof, were not protected from falling to the ground below.
