29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck was not taken out of service until it has been restored to safe operating condition.    a) Warehouse Area- The employer failed to ensure that all powered industrial trucks that were found to be in need of repair or were anyway unsafe were removed from service until they had been restored to safe operating condition.     Employees were operating the two Komatsu 225 powered industrial trucks with burnt out rear brake lights, clutch and brake pedals with missing covers, inoperable rear backup amber lights, horn, and backup alarm.
