29 CFR 1910.23(a)(8)(ii): Every floor hole into which persons could accidently walk was not guarded by a floor hole cover of standard strength and construction: a) Pavement, Parking lot: Employees were exposed to tripping hazards while walking to/from the bus to the main office, as a result of multiple holes in the pavement. These holes of various dimensions, ranged from approximately 4 feet in radius to 22 feet wide by 24 feet long. Employee exposure was observed on or about 3/14/14. Abatement Note: Permanently repair the holes in the pavement by covering and/or filling them with the appropriate filling material, thickness and strength to withstand the volume and weight of the vehicle transiting on it.

29 CFR 1910.1200(e)(1): Employer had not developed or implemented a written hazard communication program included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii): a) 1705 Wilmington Road, New Castle, DE- Employer did not develop or implemented a written hazard communication program. Observed on or about 3/14/14. Abatement Note: Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met, and which also includes the following: -A list of the hazardous chemicals known to be present using a product identifier that is referenced on the appropriate safety data sheet (the list may be compiled for the workplace as a whole or for individual work areas); and, -The methods the employer will use to inform employees of the hazards of non-routine tasks (for example, the cleaning of reactor vessels), and the hazards associated with chemicals contained in unlabeled pipes in their work areas.
