Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential struck-by hazards and caught-between hazards while moving industrial equipment with Hillman rollers.      a) For the facility at 2250 Whitfield Avenue, Sarasota, FL 34243, occurring on 03/11/2014, safe moving practices were not followed to stabilize the 5400 pound, top heavy, unbalanced, machine from falling off industrial rollers. During the move, one of the four rollers which had been installed at the corners of the injection molding machine fell off causing the machine to fall over crushing an employee.
