Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system: location: 2nd floor, lot 21-22 On or about: 3/13/14 a) Employees engaged in painting activities, were not provided with any means of fall protection. The employees were exposed to a fall of 10 1/2 feet.

The stairs did not have a hand rail.
