(a) SAY Plastics, Inc. - The employer did not implement a monitoring program when information indicated employees may equal or exceed an 8-hour time weighted average of 85 dBA as an 8 hour time-weighted average (TWA) sound level. An employee operating a CNC router and trimming parts was exposed to noise levels at an 8 hr. TWA level of 93.7 decibels and a machine operator was exposed to noise levels of 85.8 dBA as an 8 hr. TWA on or about March 25, 2014         Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
