29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition: (a) PRODUCTION SHOP - Employees performed work in the production shop where large quantities of saw dust were allowed to accumulate on the floors, equipment, horizontal surfaces, and electrical conduit and panels, thereby exposing employees to fire hazards, on or about March 14, 2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.213(h)(1): The sides of the lower exposed portion of the blade of radial saw(s) were not guarded to the full diameter of the blade by a device that automatically adjusted itself to the thickness of the stock and remained in contact with the material being cut: (a) PRODUCTION SHOP - Employees used an Original Saw radial arm saw where the blade guard was mounted upside down where it could provide no employee protection, thereby exposing employees to laceration injuries, on or about March 14, 2014. (b) PRODUCTION SHOP - Employees used a Dewalt GR-43 Radial Arm Saw(SN - 86684) where a homemade guard fashioned out of a piece of laminate material and duct tape was used as protection from the blade, thereby exposing employees to laceration injuries, on or about March 14, 2014. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
