Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502: This violation occurred on or about March 18, 2014 at a jobsite located at 5109 Eureka Lane, Sachse, Texas; where employees performing roofing work on a new residential home under construction were not protected from fall hazards of more than 9-feet to the lower level.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface, the ladder side rails were not extended at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: This violation occurred on or about March 18, 2014, at a jobsite located at 5109 Eureka Lane, Sachse, Texas; where at least two employees, who were performing roofing work, were ascending and descending from two extension ladders whose side rails did not extend at least 3-feet above the upper landing.
