29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    On or about March 18, 2014, at the construction site located at 13904 Fern Valley Road in Little Rock, Arkansas, employees were exposed to the hazard of falling approximately 10 feet, while using an extension ladder to access an upper level, and the ladder did not extend at least 3 feet above the upper landing.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.    On or about March 18, 2014, at the residential construction site located at 13904 Fern Valley Rd in Little Rock, Arkansas, employees were exposed to fall hazards greater than six feet without the use of fall protection of a personal fall arrest system.    Daniel's Framing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 315174169, citation number 1 item number 1 and was affirmed as a final order on May 7, 2011, with respect to a workplace located at #2 Hoggards Ridge in Little Rock, Arkansas.
