29 CFR 1910.215(b)(9): Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, were not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel. The maximum angular exposure above the horizontal plane of the wheel spindle as specified in paragraphs (b)(3) and (4) of this section shall never be exceeded, and the distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top shall never exceed one-fourth inch. a. B.W. Grinding Co. located in Lyons, Ohio: On or about March 19, 2014 the employer did not ensure that employees who operated the pedestal grinder in the finishing department were protected from the hazards of wheel breakage in that the upper peripheral guard was adjusted to no more then one-fourth inch from the abrasive wheel. B.W. Grinding Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.215(b)(9), which was contained in OSHA inspection number 444493, citation number 1, item number 6 and was affirmed as a final order on November 26, 2012, with respect to a workplace located at 15048 CR 10-3, Lyons, Ohio 43533.
