29 CFR 1910.1000(c): The employer did not ensure that an employee's exposure to any substance listed in Table Z-3 in any 8-hour work shift of a 40 hour work week did not exceed the 8-hour time weighted average limit given for that substance in the table: a. On or about 3/26/2014, Mason321HCG near Shattuck, OK, a sand king employees personal 8-hour time-weighted average (8-Hr TWA) exposure to respirable dust containing 58% Crystalline Silica Quartz was 0.313 mg per cubic meter of air during well fracturing operations. This exposure exceeded the OSHA 8-Hr TWA Crystalline Silica Quartz PEL of 0.167 mg per cubic meter of air by 1.87 times for respirable dust containing % Crystalline Silica Quartz. The personal air monitoring consisted of one sample collected during a 247 minute sampling period on 3/26/2014. The remaining un-sampled time of 233 minutes was included in the 8-Hr TWA as a zero exposure period. b. On or about 3/26/2014, Mason321HCG near Shattuck, OK, a blender employees personal 8-hour time-weighted average (8-Hr TWA) exposure to respirable dust containing 50% Crystalline Silica Quartz was 0.346 mg per cubic meter of air during well fracturing operations. This exposure exceeded the OSHA 8-Hr TWA Crystalline Silica Quartz PEL of 0.192 mg per cubic meter of air by 1.80 times for respirable dust containing 50% Crystalline Silica Quartz. The personal air monitoring consisted of one sample collected during a 266 minute sampling period on 3/26/2014. The remaining un-sampled time of 214 minutes was included in the 8-Hr TWA as a zero exposure period.

29 CFR 1910.1000(e): Feasible administrative or engineering controls were not determined and implemented to achieve compliance with the limits prescribed in 29 CFR 1910.1000(a) through (d): On or about 3/26/2014, Mason321HCG near Shattuck, OK, feasible administrative controls and engineering controls were not first determined and implemented for well fracturing operations where two employees were exposed to crystalline silica in excess of the OSHA 8-Hr TWA Crystalline Silica Quartz PEL. General methods of control applicable in these circumstances include, but are not limited to the following: 1. Substitution of silica containing proppant with non-silica containing proppant such as ceramics 2. Enclose dust generating equipment such as conveyors, chutes, and blender hoppers with stilling curtains 3. Utilize wet methods (water misting) near dust generating equipment areas 4. Remotely operate sand king conveyors outside of crystalline silica exposure areas 5. Use video equipment to remotely monitor the blender hopper 6. Increase the distance employees stand to monitor the blender hopper 7. Limit the time period any one employee works at the blender

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a. On or about 3/26/2014, Mason321HCG near Shattuck, OK, during well fracturing operations an emergency eye wash was not immediately available to a blender employee who poured containers of corrosive materials including but not limited to Ferrotrol which was a corrosive liquid. Employees were exposed to eye injury hazards. Baker Hughes Oilfield Operations, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.151(c), which was contained in OSHA inspection number 315848176, citation number 1, item number 1, and was affirmed as a final order on 1/11/2012, with respect to a workplace located at Monroe 10-14 #6-28H33, Bee Branch, AR 72013.
