29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c): (a) Jobsite - on or about March 19, 2014 - the excavation was not provided with an adequate protective system to prevent a cave-in hazard. The excavation measured approximately 35 feet long by 17 feet wide by 9 feet deep.
