On or about March 1, 2014, at the west side of the building, three employees were exposed to the hazard of being struck in the head by falling objects in that they were not wearing hard hats while picking debris on the ground.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a. On or about March 1, 2014, at the west side of the office building, six employees installing roof shingles on a 7:12 pitch roof were not protected from an 11-ft 3-in fall hazard by the use of a fall protection system. b. On or about March 1, 2014, at the northwest side of the office building, one employee installing weather proofing material on a 7:12 pitch roof was not protected from a 10 ft. fall hazard by the use of a fall protection system. Rogelio Rios Matamoros, d.b.a. Rios Roofing, was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 314031196, citation 01, item 1, issued on October 14, 2010, with respect to a workplace at 1338 N. Aksarben Ct., Wichita, KS, 67235. The final order date was November 9, 2010.
