29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) Facility, 5350 Campbells Run Road, Pittsburgh, PA: On or about 03/10/14 a lockout or tagout device was not affixed to the energy isolating device on the Cosmos RF Heat Press #3 when an employee was making an adjustment to the wing nut for the top stop. The employee was standing on a step stool to reach the wing nut and leaned into the machine and grazed a stop button with their body which then activated the ram and caused it to rise. The employee's pointer and middle fingers were pinched between the top bar and stop causing severe lacerations and crushing injuries.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by the heating of the ram arm: (a) Facility, 5350 Campbells Run Road, Pittsburgh, PA: On or about 03/19/14 no guard was in place on the #2 Heat Sealer to protect the operator from contact with the energized ram and heated contact bar. Employees are exposed to electrical shock and burns from potential contact with the unguarded parts.
