29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) On or about March 19, 2014, the employer did not have a lock out program for the control of the hazardous energy, which includes machine specific procedures, training, and periodic inspections, while performing maintenance, servicing, and cleaning machines such as, but not limited to, Haas CNC machines. Failure to lock out these components while these tasks are being performed exposes employees to caught-in hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about March 19, 2014, the employer did not assure that the employees operating the Hass CNC machines were protected from the rotating tool and the point of operation. The employees can open the door when the machines are running due to a safety interlock not being present or being manually disconnected. AVD changed to: a) On or about March 19, 2014, the employer did not implement and provide training on adequate alternative safety measures to be used during the setup process on the Haas CNC mills.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: a) On or about March 19, 2014, the employer did not provide information and training to employees in the safe handling and use of hazardous chemicals including, but not limited to, Rust Preventative (G&G Rust SC-18). Employees were not aware of the Personal Protective Equipment (PPE) requirements, first aid measures, and potential health effects of exposure to these chemicals.
