29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft ends did not present a smooth edge and end and projected more than one half the diameter of the shaft:  (a) Filled Bar Wrapping, Main Distribution Line Conveyor, Drive End, E Side - An unguarded horizontal shaft end, 19 inches above the floor, on the E side at the drive end of the Main Distribution Line conveyor had a diameter of 3.25 inches and projected 1.75 inches, exposing employees to a caught in rotating parts hazard, on or about March 19, 2014.  (b) Filled Bar Wrapping, Main Distribution Line Conveyor, Drive End, E Side - An unguarded horizontal shaft end, 3 feet 3.5 inches above the floor, on the E side at the drive end of the Main Distribution Line conveyor did not have a smooth edge because of the two key ways in this shaft end, exposing employees to a caught in rotating parts hazard, on or about March 19, 2014.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
