Gateway Recycling and Waste Reduction, Inc. located at 926 Dearborn Avenue, Toledo, OH; On or about March 19, 2014, the employer did not ensure employees were protected while changing wire spools on the spool platform. Employees are required to access the platform to move empty spools to make room for the new spools, and are exposed to falls of up to 8 feet to the concrete below.      c.)    Gateway Recycling and Waste Reduction, Inc. located at 926 Dearborn Avenue, Toledo, OH; On or about March 19, 2014, the employer did not ensure employees were protected while preparing to perform maintenance on the cardboard baler (IPS Model #AT-1080-HS-100S, Serial #3203). Employees are required to access a hopper door above an electrical panel and are exposed to falls of up to 7 feet to the concrete below.
