29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:     a.)    Gateway Recycling and Waste Reduction, Inc. located at 926 Dearborn Avenue, Toledo, OH; On or about March 19, 2014, the employer did not ensure employees were protected while unjamming the cardboard baler (IPS Model #AT-1080-HS-100S, Serial #3203). Employees are required to climb up the conveyor of the baler and are exposed to falls of approximately 25 feet inside the baler.      b.)    Gateway Recycling and Waste Reduction, Inc. located at 926 Dearborn Avenue, Toledo, OH; On or about March 19, 2014, the employer did not ensure employees were protected while changing wire spools on the spool platform. Employees are required to access the platform to move empty spools to make room for the new spools, and are exposed to falls of up to 8 feet to the concrete below.      c.)    Gateway Recycling and Waste Reduction, Inc. located at 926 Dearborn Avenue, Toledo, OH; On or about March 19, 2014, the employer did not ensure employees were protected while preparing to perform maintenance on the cardboard baler (IPS Model #AT-1080-HS-100S, Serial #3203). Employees are required to access a hopper door above an electrical panel and are exposed to falls of up to 7 feet to the concrete below.

Employees are required to sort material, operate the baler, and transport material and are exposed to suspended paper material.

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment:     a.)    Gateway Recycling and Waste Reduction, Inc. located at 926 Dearborn Ave. Toledo, OH; On or about March 19, 2014, the employer did not assure each employee performing servicing or maintenance applied a lock to the lockout point. Employees are required to lock out the cardboard baler before walking up the conveyor to clear the jam. Only one employee places a lock when two employees perform the work exposing the employees to accidental energization of the baler.
