29 CFR 1910.23(a)(5): Every pit and floor opening was not adequately guarded by standard railing or any other effected means: a) On April 3, 2014, in R-bay, the ladle pre-heater elevated platform that was 8 feet above the ground had angular openings in the floor grating on the southeast and southwest sides of the ladle that measured 14 inches by 5 feet. b) On April 3, 2014, in R-bay, the ladle repair stand elevated platform that was 8 feet above the ground had angular openings in the floor grating on the southeast and southwest sides of the ladle that measured 18 inches by 6 feet. c) On April 8, 2014, at the melt shop electric arc furnace, southwest side; the west side of the slag door had an angular opening in the floor plate that measured 3 feet by 2 feet wide and approximately 15 feet above the basement level. d) On April 10, 2014, at the multi-stand pipe mill, billet cross transfer table from the re-heat furnace there were multiple sections of floor grating missing on the elevated platform along the eastside of the transfer table. e) On April 16, 2014, at the multi-stand pipe mill, #1 piercer mill flume system there was a floor opening that measured 15 inches by 17 inches wide and approximately 10 inches deep over the running water. f) On May 14, 2014, at the FQM, hot metal area the billet saw chip conveyor was missing the floor plate, exposing a floor opening that measured 2 feet wide, 4 feet long and 20 inches deep.

29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded: a) On April 8, 2014, at the melt shop electric arc furnace, south side; the east side of the slag door had a rectangular opening in the floor plate that measured 7 inches wide by 8 feet long and approximately 15 feet above the basement level. Additionally the west side of the furnace had scaffold picks laying across the floor level with varying openings of 8 inches to 30 inches wide by approximately 30 feet long. b) On April 9, 2014, the melt shop continuous caster straightener section had three floor gratings misaligned with openings approximately 6 inches wide by 6 feet long. c) On April 16, 2014, at the multi-stand pipe mill, mill stand, between stands 2, 4 and 6; the north side of the six mill stand had openings at the mill area between the elevated platform floor plates and the mill housings. The openings measured approximately 8 inches wide by 5 feet long with variations of piping and machinery that lead to the basement area approximately 15 feet below. d) On April 16, 2014, at the multi-stand pipe mill number 4 hydraulic valve stand for the equalizer, sizing and cooling segments there were floor gratings misaligned with openings that measured 8 inches by 18 inches wide and 2.5 inches by 3 feet wide, both approximately 18 inches deep. e) On May 14, 2014, at the FQM, hot metal; the billet saw operator elevated platform had an opening at the edge of the floor plate that measured 5.5 inches wide 5 feet long and 5 feet deep. f) On June 9, 2014, at the FQM, finishing area there was a floor plate misaligned on the west side of the billet elevator to the AF furnace with an opening measurement of 2 inches wide by 3 feet long.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: Note: Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard: a) On March 31, 2014, the melt shop scrap bay #86 overhead crane landing, at the bottom of the standard guardrail, had an angular opening at the side of 30 inches to 14 inches at 5 feet long and 15 feet above the ground. b) On March 31, 2014, the melt shop lime alloy hopper was 10 feet wide and 5 feet deep at the edge. c) On April 2, 2014, the melt shop, C bay, north wall crane runway horizontal lifeline was sagging approximately 2 feet. d) On April 2, 2014, the melt shop continuous caster had two sequencer cars with an unguarded elevated service platform approximately 30 feet long, 2 feet wide and 13 feet above the work floor. e) On April 2, 2014, the melt shop continuous caster, south elevated platform that was approximately 20 feet above the ground had a 10 inch wide section of the guardrail system that was missing at the east side and next to the scrap box. f) On April 3, 2014, in R-bay, the BM overhead crane cab had an operator boarding platform on the west side that did not have a guardrail at the north side. g) On April 8, 2014, at the melt shop, electric arc furnace, employees were performing work on the top of the furnace at approximately 15 feet above the work floor. An employee was wearing a fall harness with two shock absorbing lanyards attached to a horizontal lifeline that was lying on the roof floor and at the feet of the employee. The horizontal lifeline attachment is performed when the employee steps off of the portable staircase and walks across the furnace roof to attach the lifeline end to the structural column. While this task is performed, the employee is not attached to the horizontal lifeline and could fall off of the furnace roof. h) On April 8, 2014, at the melt shop ladle refining furnace, east side there was an elevated platform approximately 8 feet above the ground and next to the mast arms that did not have a standard guardrail system. Additionally, one of the vertical ladder sides were cut off at the top at the leading edge of the open sided platform. i) On April 8, 2014, at the melt shop continuous caster east tundish pre-heater, on the south side there was an elevated platform that was 49 inches high with an open sided edge at 36 inches wide. j) On April 10, 2014, the multi-stand pipe mill piercer drive room roof was approximately 12 feet above the work floor and approximately 40 feet long. There was a fixed vertical ladder attached to the side wall of the building for access to the roof. Employees walking/working on the roof were not able to use fall protection and there was no standard guardrail system around the perimeter of the roof. k) On April 16, 2014, at the multi-stand pipe mill, #1 piercer mill lube basement; the lube reservoir tank had a fixed vertical ladder attached to the side of the 8 foot high tank. Employees working on top of the tank were not able to use fall protection and there was no standard guardrail system around the perimeter of the tank. l) On April 16, 2014, at the multi-stand pipe mill, mill stand; the northeast corner of the mill stand had an elevated platform made of grating approximately 10 inches wide and 5 feet long for access to the step up platform on the east side with potential to fall off the unprotected west side onto the machinery below including the basement approximately 15 feet below. The elevated platform did not have a standard guardrail or anchorage for fall protection. m) On May 14, 2014, at the FQM, rotary furnace at the top of the combustion air duct area there was an elevated walkway that was 16 inches wide. The elevated walkway was a pass through between the duct work. The elevated walkway was approximately 20 feet above the ground level and open at the radius of the vertical duct work. The elevated walkway did not have a standard guardrail system. n) On April 3, 2014, the R-Bay gate make up roof was used for accessing the overhead crane and servicing the HVAC system. The north side of the roof had a standard guardrail system, the south side was against the building wall, the east and west sides of the roof were open and did not have a guardrail system. The two sides were approximately 10 feet above the ground and 15 feet long.

There was a steel alloy chain used as a segment barrier.

Good safe practices in the use of portable metal ladders were not implemented in that, the top of the ladder was not three feet above the top edge of the ladle nor was the ladder secured at the top. b) On April 8, 2014, at the melt shop electric arc furnace, northwest corner there was an extension ladder in place for the employees working on top of the cylindrical tank that was approximately 10 feet above the ground. Good safe practices in the use of portable metal ladders were not implemented in that, the top of the ladder was not three feet above the top edge of the tank nor was the ladder secured at the top.

The electrical toggle switch was missing the front cover plate. b) On April 16, 2014, at the multi-stand pipe mill, valve stand #13 there was an electrical elbow fitting with the front cover plate missing. c) On April 16, 2014, at the multi-stand pipe mill, on the south side of #13 valve stand attached to the east wall there was an electrical junction box with the cover plate damaged and open at the bottom. d) On April 16, 2014, at the multi-stand pipe mill, #1 piercer cap operator station elevated platform there were two electrical elbows at the floor level on the north side that were damaged and open. One face plate cover was missing and the other was damaged and wired to the fitting with the face plate cover half protecting the opening.

29 CFR 1910.253(e)(6)(i): Pressure reducing regulators were not used for the gas and pressure for which they were intended: On March 31, 2014, at the melt shop, on the west side of the electric arc furnace pulpit, there was an oxygen and natural gas cutting torch/hose system. The acetylene fuel gas regulator and gauge attached to the natural gas line was not designed for the purpose. Additionally both the regulator gauges were coated and not clear or legible. NOTE: Acetylene regulators and gauges are specifically designed for acetylene fuel, constructed with a spring setting based on the volatile nature of the fuel above 15 psi. and the cubic feet per hour delivery. Propane fuel regulators are an acceptable regulator with natural gas, based on the similarity as a fuel gas and the delivery pressure.
