29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems: a) At the jobsite, inside the trench, on or about 3/19/14: Employees were exposed to hazards from falling material where spoil was placed to the edge of the trench, no protective systems were provided for employees inside the trench with depths of approximately 10 feet below grade, and no competent person inspections were conducted that identified these hazards for trench failure. Abatement certification must be submitted for this item.
