29 CFR 1910.333(a)(1): The employer failed to deenergize live parts to which an employee may be exposed before the employee works on or near them:  Location: 450 Oak Tree Avenue, South Plainfield, NJ 07080. The employer did not ensure that electrical equipment was deenergized prior to employee(s) working on or in it. Thereby exposing employee(s) to shock, burns and electrocution while working with energized parts.     a)    Employee(s) were working on, and in, an energized 1200 Amp Main Distribution Panel, when arc flash occurred resulting in the employee receiving burns.   Incident occurred on or about 02/20/14.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there are potential electrical hazards shall be provided with, and shall use, electrical protective equipment that is appropriate for the specific parts of the body to be protected and for the work to be performed:   Location:  450 Oak Tree Avenue, South Plainfield, NJ 07080 The employer did not ensure that proper electrical protective equipment, such as but not limited to rated electrical gloves and flame retardant clothing,  was used or provided to employee(s) working on an energized electrical equipment. Thereby exposing employees to shock, burns and electrocution hazards.    a) Employee(s) working on an energized 1200 Amp Main Distribution Panel, when arc flash occurred resulting in the employee receiving  burns while working with energized parts  Incident occurred on or about 02/20/14
