29 CFR 1910.106(e)(2)(iv)(d): Flammable liquids were not drawn from or transferred into vessel(s), container(s), or portable tank(s) within a building through a closed piping system, from safety cans, by means of a device drawing through the top, or from a container or portable tank by gravity through an approved self-closing valve: a/ Frutarom/Branchburg NJ Site/Production/Mixing Area: Ethyl Alcohol/190 Proof Ethanol (Flash Point 55.4 F to 60.8 F/Boiling point 173.38 F) Class 1B, Category II flammable liquid. Ethyl alcohol (including but not limited to) is dispensed from 55 gallon drums (horizontal) without the use of a self-closing valve. Observed on/or about 04/01/2014.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by falling hazard: a) Frutarom/Branchburg NJ Site/Production/Mixing Area: Four electrically powered vertical mixing tanks with vertical agitators were not adequately guarded to prevent employees from falling into the mixers during dispensing and washing operations. The covers provided did not have a grate under them with openings only large enough to allow for hoses/brushes for washing the insides and for the raw material being dispensed. Observed on/or about 04/04/2014.
